1. Introduction & Consent to Telehealth
By using our telehealth services, you consent to receive medical care from our providers via remote electronic communications, such as video, audio, and secure chat. You understand that:
Telehealth involves the use of secure, HIPAA-compliant interactive audio and video equipment.

A virtual evaluation may limit our ability to fully diagnose conditions compared to an in-person physical exam.

If our providers determine your child's condition cannot be safely or accurately diagnosed virtually, you will be required to bring your child to an in-person clinic or seek immediate emergency care.
2. Parental / Guardian Responsibility

Account Holder: The account holder and primary contact for any patient under the age of 18 must be the child's parent or legal guardian.

Presence of Guardian: A parent or legal guardian must be physically present in the room with the minor child during the entirety of the telehealth appointment.

Accurate Information: You agree to provide accurate, up-to-date medical history, current symptoms, and a correct physical location at the start of every appointment.
3. Privacy, Confidentiality, & Records

HIPAA Compliance: All telehealth sessions are protected by the same HIPAA laws and confidentiality regulations that apply to traditional in-person medical visits.

No Unauthorized Recording: Neither the clinic nor the patient/parent is permitted to record, screenshot, or distribute the audio or video of the telehealth session without prior written consent.

4. Technical Requirements & Connectivity

Environment: You are responsible for ensuring the appointment takes place in a private, quiet, and well-lit setting.

Equipment & Connectivity: You are responsible for ensuring you have a working device (computer, tablet, or smartphone) with a stable internet connection.

Disconnections: If the video or audio connection fails during the visit, our clinic staff will reach out via phone to troubleshoot or reschedule.
5. Financial Responsibility & Insurance

Charges: You understand that you are financially responsible for any charges associated with your child’s telemedicine visit.

Insurance Coverage: While we verify benefits, it is the parent's responsibility to know their insurance carrier's telehealth coverage policies. You will be billed for any deductibles, copays, or non-covered services.
6. Emergency & After-Hours Policy
Telehealth visits are not for medical emergencies. If your child is experiencing a life-threatening emergency, severe respiratory distress, or ingestion of toxic substances, immediately call 911 or go to the nearest emergency room.
7. Termination of Services
You have the right to withhold or withdraw consent to telehealth services at any time, which will revert your child's care to traditional in-person services without affecting future care benefits. The clinic also reserves the right to terminate telehealth services if the parent or patient violates these terms or engages in abusive behavior.
